Modified Barium Swallow Study

Key Points

  • MBSS evaluates swallowing physiology in real time under fluoroscopy.
  • MBSS is commonly performed with speech-language pathology to assess swallow safety and physiology during bolus transit.
  • It identifies aspiration risk and guides safe texture/fluid recommendations.
  • Results support decisions on oral diet progression versus alternative nutrition routes.

Pathophysiology

Swallowing impairment can cause aspiration, malnutrition, dehydration, and recurrent pulmonary complications. MBSS visualizes bolus movement and airway protection to identify where swallowing fails and what compensatory strategies improve safety.

Classification

  • Indication pattern: Suspected dysphagia, coughing/choking with intake, recurrent aspiration risk.
  • Outcome pattern: Safe oral intake, modified consistency needs, or non-oral nutrition recommendation.

Nursing Assessment

  • Assess swallowing symptoms, weight trend, hydration status, and respiratory complications.
  • Assess current diet tolerance and prior aspiration events.
  • Assess readiness for interdisciplinary swallow evaluation referral.

Nursing Interventions

  • Coordinate MBSS referral and communicate pretest swallowing concerns clearly.
  • Implement posttest recommendations for diet texture, liquid thickness, and positioning strategies.
  • Document MBSS findings in nutrition planning notes and align handoff communication with current swallow-safety level.
  • Monitor for aspiration signs after diet changes and escalate deterioration promptly.
