Antimicrobial Stewardship
Key Points
- Antimicrobial stewardship is a coordinated program to promote appropriate antimicrobial use.
- Core goals are to improve client outcomes, reduce microbial resistance, and decrease spread of multidrug-resistant organisms.
- Inappropriate prescribing and incomplete therapy accelerate resistance.
- Resistant infections are linked to longer, higher-cost hospital stays and higher mortality risk.
- Professional nursing advocacy includes safeguarding antibiotics and supporting comprehensive prevention against inappropriate antimicrobial use.
Core Framework
Antimicrobial stewardship aligns prescribing, administration, monitoring, and education so each client receives the right antimicrobial exposure for the confirmed or most likely pathogen while avoiding unnecessary use.
Resistance Drivers to Identify
- Antibiotic use for nonbacterial illness patterns (for example routine viral symptoms).
- Antimicrobial selection that does not match the infectious organism.
- Early discontinuation or noncompletion of prescribed therapy.
Nursing Assessment and Action
- Review whether the suspected pathogen type supports antimicrobial use.
- Explain that antibiotics treat bacterial infections, not routine viral illness patterns.
- Reinforce full-course completion when antibiotics are prescribed.
- If adverse effects (for example nausea or diarrhea) occur, instruct clients to contact the prescriber instead of self-discontinuing.
- Escalate concerns for resistant-organism patterns (for example MRSA or VRE) and align precaution workflows.
- Support facility stewardship standards and interdisciplinary review.
Related Concepts
- antibiotics - High-volume antimicrobial class requiring strict use controls.
- multidrug-resistant-organisms - Stewardship reduces resistance pressure and spread.
- healthcare-associated-infections - Stewardship supports HAI prevention and safer outcomes.
- culture-and-sensitivity-testing-in-infection-management - Diagnostic matching supports targeted therapy.
Self-Check
- Which prescribing or adherence behaviors in your setting may increase resistance pressure?
- How does stewardship improve both individual client outcomes and population-level infection control?
